Welcome Baby: Pathways to Resilience programs are designed to help you feel supported throughout your pregnancy and postpartum experience. We want to see your family reach your goals, provide physical items that can help you and your baby, and help you to connect and build a social support network.

Welcome Baby Program
  • At least 1 postpartum visit with a nurse to address any concerns or questions you have about your newborn or your birth recovery, and initiate connections to community resources.
  • Up to 2 additional follow-up visits from our program staff to fulfill any individual care needs.
  • Basic physical client support items, e.g. diapering supplies.
  • Referral to Pathways to Resilience as applicable.
Pathways to Resilience Program
  • Both all the services that a Welcome Baby Client can receive, and individualized case management and care plans for pregnant or postpartum individuals and their immediate household, including:
    • Monthly visits with program staff,
    • care coordination,
    • transportation support,
    • connection to community resources,
    • parent education,
    • participant gatherings, and access to additional physical client support items, e.g. childproofing supplies and strollers.
  • Eligible clients receive services for at least 6 months, up to 18 months.

Our program eligibility is NOT based on income. Service delivery may vary.

First 5 Humboldt has partnered with your local providers to provide care that can meet you where you are at.

The way to Welcome Baby: Pathways to Resilience was paved by a previous program called, “Road to Resilience,” which was a five year grant-funded program that provided care to pregnant and postpartum families through United Indian Health Services, Inc. When it ended, we looked for ways to expand and continue reaching pregnant and postpartum families. To this end, we were able to secure another grant which allows us funding through June 2027.

We are contracted with five local healthcare agencies so that you can get Welcome Baby: Pathways to Resilience services close to home, from staff members that you know and trust. These partners offer nurse and navigator* visits to provide case management and support to pregnant and postpartum families.

Together with our partners we coordinate and fund holistic, evidence-based, and flexible curriculum/trainings; meaningful physical items for participants; and community-building events that strengthen connection and support.

*Navigator positions may also be known as Community Health Representatives or Community Health Workers.

United Indian Health Services, Inc.

United Indian Health Services (UIHS) was the cornerstone to our prior program (Road to Resilience) and we are thrilled to continue to work with them.

UIHS began offering Welcome Baby: Pathways to Resilience services in May 2025 to eligible UIHS clients. Services include home visits and both Welcome Baby and Pathways to Resilience program offerings.

K’ima:w Medical Center

Our contract with K’ima:w Medical Center has been signed and we are so excited to be working on what service delivery will look like. More info coming soon!

County of Humboldt Public Health Nursing

Public Health Nurses began offering Welcome Baby: Pathways to Resilience services in May 2025 to connected clients. Services include home visits and Welcome Baby program offerings only, with referrals to other programs and partners for clients who wanted continued support.

Public Health Nursing staff are committed to Humboldt County’s perinatal health, and offer several of their own programs, including Nurse Family Partnership (NFP). Find out more about Public Health Nursing at their Website.

Redwoods Rural Health Center, Inc.

RRHC began offering Welcome Baby: Pathways to Resilience services in October 2025. Services include Welcome Baby and Pathways to Resilience visits at the Health Center.

Located in Southern Humboldt, Redwoods Rural’s perinatal team cares deeply about our community, and jumped into this program for the opportunity to continue to wrap as many services around families as possible. Find out more about their team here.

Perinatal is a term that encompasses various stages and events related to pregnancy, labor, delivery, and the postpartum period. 

Program Performance

The Welcome Baby programs began limited client service in 05/2025. Due to services being so recently started, no public data is available regarding program performance at this time. Check in again later for more details.

Event Feedback

Welcome Baby programs were a majority contributor to the Community Baby Shower that First 5 Humboldt hosted in July 2025. Following this event, thirty-five attendees completed our feedback survey and the data highlights from this survey can be downloaded below.
