Keeping Connected: Technology in Practice

One thing that has held true in the last few months is that our strength builds from the connections we make and that even one relationship can make a huge difference. In today’s world these ties aren’t limited to the face-to-face; the digital plays an increasingly large role.

In the last few months, several First 5 Humboldt staff members have been trained in the North Coast Care Connect’s Community Information Exchange (NCCC’s CIE), supported by the North Coast Health Improvement and Information Network (NCHIIN). This platform allows us to securely share information for the purposes of care coordination and referrals to other participating CIE partners. The platform is steadily growing and we are looking forward to offering closed loop referrals and connections within the CIE for any of our clients who consent to and opt into this service.

We also want to make it easy for the public to get connected to our services and have been exploring ways to build access through our Apricot database. The Welcome Baby: Pathways to Resilience program is now accepting eReferrals this way (http://www.tinyurl.com/F5HWBRef)! Using this link, clients can self-refer or be referred by their service providers at any time of day. Referred clients will be contacted by program staff, screened for eligibility, and able to access services as soon as they are available.

Community members can stay connected to First 5 Humboldt events and local spotlights via our social media – with content now available across platforms, including Facebook, Instagram, Spotify, Apple Podcasts, and YouTube.
