Welcome Baby: Pathways to Resilience

The Welcome Baby program offers at least one nurse home visit after birth to all newly birthing parents with the option of two follow-ups. In addition, pregnant or newly parenting people who screen positive in the 4Ps are eligible to receive up to eighteen months of case management and monthly visits with a Community Health Representative aimed around goal setting and supporting the family. The program is currently being implemented at United Indian Health Services, Redwoods Rural Health Clinic, and through Public Health Nurse Family Partnership. We are also excited to be working with Open Door Community Health Centers to bring Welcome Baby through Centering Pregnancy early next year and in supporting the development of Welcome Baby: Pathways to Resilience at K’ima:w Medical Center. Individuals can talk with their provider if they are affiliated with a Welcome Baby location or can self-refer on our website.
