Welcome Baby: Pathways to Resilience Services Begin at United Indian Health Services

We are so excited to announce Welcome Baby: Pathways to Resilience program services will be available at United Indian Health Services on May 12th. It feels very fitting to begin rolling out services with UIHS, as this program was built and created from the collaborative efforts over the last five years that UIHS and First 5 Humboldt did on the Road to Resilience program. Over the last month and a half, we have been working together to prepare staff to offer this program to the community by providing weekly training to build familiarity and confidence with the expanded program. With this expanded program, Community Health Representatives (CHRs) will support pregnant and parenting families who may be impacted by substance use with wrap-around, in-home, case management care and aim to improve the overall health and wellness of the families they serve. The second component of our program will aim to support any family who has given birth by offering one postnatal home visit by a nurse and follow up care with a CHR as needed. Currently the services described are exclusive to clients who receive care at UIHS or are eligible to receive care at UIHS but will be available at our other partnering agencies soon.
